Position Responsibilities:

  1. Develop and deliver family centered casework services to children and families.
  2. Provide Aftercare/Transition services for residential clients entering foster care or returning to biological families, and to meet mutually established goals that promote stability and permanency in the best interest of the child.
  3. Work with the assigned permanency worker, child, and family to ensure all post discharge treatment and services are being provided.
  4. Document all services delivered to comply with agency, state, and federal regulations and requirements.
  5. Complete the following reports: Clinical Case Review, Administrative Case Review, Court Reports, Discharge Summary, Social Security Income forms, and any other paperwork as assigned.
  6. Attend child and family staffings, or special staffings, case staffing sessions, giving input into the decision making process regarding service delivery and goal setting with families.
  7. Attend court hearings, case reviews (ACR's), case staffings, or other activities necessary to coordinate client services.
  8. When necessary, travel to deliver services in the family's home.
  9. Participate on the Guardian Angel Home Leadership Team.

Requirement include:

  • Bachelor degree required. Highly preferred in social work or human service degree as per current DCFS licensing standards (Preferred to have one year social work experience)
  • Must have ability to pass DCFS background check
  • Travel is required. Must have valid driver's license, three years' driving experience, and clean driving record
